Consent is key, especially if you have the capacity to make decisions. If not, a Power of Attorney or a best interest meeting will be required to proceed with the assessment.
The NHS Continuing Healthcare Checklist
This checklist, completed by a healthcare professional familiar with your care needs, evaluates 11 care domains, ranging from breathing and nutrition to mobility and medication. Each domain is assessed with a grade from A (most significant need) to C (least significant).
The checklist results will guide the decision on whether a full assessment for NHS Continuing Healthcare is necessary.
Completion of the Checklist
The professional will use the checklist to match your needs against specific criteria. They’ll record a brief description of your needs in each domain. The number of As, Bs, and Cs are then tallied to determine eligibility based on specific criteria.
The Principle of Well-Managed Needs
An important aspect of CHC funding is recognizing that well-managed needs are still needs. Even if an intervention successfully manages a need, it doesn’t mean the need no longer exists.
What Happens After the Initial Assessment?
